- Print and complete the form on the next page.
- Make your check payable to Chestatee High School Band Booster Inc.
- Mail the form and your check to the following address:

- Once we receive your check, we will email you to confirm it was received. Our primary communication with you will be through email to make it easy and immediate for you to hear from us. In addition, you will receive a letter from CHS Band Boosters confirming your donation amount for tax purposes.
- In August, we will send your VIP package with the benefits listed above based on your sponsorship level! We will update our band website in August with the new sponsors!
